As Greene clocks in at County General for his shift, the first spate of ambulances arrive from a shooting incident at a nearby foster home, where a gunman has fired at workers and children.

As the corridors fill up with the wounded, Greene realises the gunman is the father of a young patient he had placed under a care order. When the social worker who co-signed the order is shot at her apartment, Greene guesses that Elizabeth and baby Ella are next on the gunman's list.

Meanwhile, Cleo is injured in an accident that may have devastating consequences, Weaver lays down the law to Romano, and Carter makes a confession to Abby.
