E111 forms invalid from 2006

UK travellers were today warned they have less than a month to get the new form of health insurance card for trips to Europe.
The existing E111 form, entitling UK residents to free or reduced cost health care in most European countries, becomes invalid on January 1, 2006.
The Department of Health today urged those wanting to travel over the Christmas and New Year period to apply at once for EHIC (European Health Insurance Card) which is replacing the E111.
A spokesperson said: 'If you become ill or get injured on your New Year break, the last thing you want to be thinking about is paying unnecessary medical bills.
'E111 becomes invalid on 1st January so it's vital that from then, anyone travelling in Europe carries an EHIC - as well as having comprehensive travel insurance.'
People can apply for the EHIC online, by phone at 0845 606 2030, or by post to EHIC Applications, PO Box 1115, Newcastle upon Tyne, NE99 1SW.
Applicants must submit name, date of birth and NHS or National Insurance number.
