I can grant you distant access to my laptop using teamviewer to resolve directly the problem from my PC.
If not, in which website should I open a support ticket?

Problem resolved thanks to the assistance given through support system available from the backend of the plugin
I don’t know why the plugin authors do not assist me through this official forum.
I would like to share the solution with all the world.
In my case, I have 2 plugins installed:
MailChimp Forms by MailMunch
and mailmunch plugin
The solution consist in keeping one of them not both of them.
