Have installed recently any plugin that may cause this CSS issue?
Is this happening also with all you wordpress bulk actions or just in the plugin page?
If you do not have a big number of installed plugins, one first solution consists of deactivating your plugins one by one and check each time if the problem is still happening.

I am using only 1 plugin named autoptimize, which combines and minifies css and js. But that is being used from a long time and never had problem with any plugins (including your’s).
i have a total of 9 active plugins and i have already tried the deactivation approach you mentioned. Even after i had deactivated all the plugins, still then the bulk action option is not visible.
If you want i can give you access to my admin panel as well.

moreover autoptimize does not optimize CSS/ JS on admin-pages.
