I am sorry to hear about the issue you are facing with Forminator forms. The issue that you notice could be due to multiple factors affecting the form. However, when I made a test on the page you’ve shared, the form worked just fine and the submission was successful. Could you please confirm if you were able to resolve the issue already?
If not, could you please confirm if the issue is still replicated on the Job Application form (as I tested the one on the contact page which seems to be the appointment schedule form).
Since the Admin-ajax response triggers a 403 Forbidden error, and you’ve already tried resetting the .htaccess file, which didn’t help, could you please verify if the folder permissions are set correctly for the site files on your server?
The easiest way to check if the file permissions are looking good is to navigate to the WP Admin >> Tools >> Site Health >> Info tab >> Scroll down to Filesystem Permissions. They should look as https://monosnap.ai/file/LTtOq0WRozZq5jHxumzhv3eh4HtWnN
Further, in case the filesystem permissions look good, could you please share a screenshot of the site health >> Status tab to check if it shows any issues?

I’m still experiencing an issue with one of my forms — “Application Form – Join Our Team.”
If you scroll down on this page: https://eifcenter.com/contact-us/ you can test it directly. The form fails every time with the same error message.
I’ve already tried the following:
- Checked file permissions — everything looks consistent and correct.
- Reset
.htaccess and re-saved permalinks.
- Deactivated other plugins and switched to a default theme for testing.
- Even newly created forms show the same issue.

Thank you for your additional input. I attempted to replicate the issue on both forms, but unfortunately, I was unable to reproduce it on my end.

We need additional information on this case. Would you mind testing the same things using a mobile browser with mobile data? It could be specific to your computer or a browser extension causing the issue. That is possible. Also, you can share the Device OS, Browser name, and version so we can set up the same environment to test the form.
Lastly, when you get a 403 error in the WP-Ajax request, please navigate to the preview tab as described in the screenshot here: https://monosnap.ai/file/WWVTKtEclDmuT9wlDtA1DTX4QrJXhP to see if it provides any additional information about the 403 error.
