Home health agencies provide skilled nursing care, occupational, speech, or physical therapy, and various medical social services to older adults and other vulnerable populations in their private environment. In addition, they often operate coordinated post-discharge services for patients leaving hospitals.i
To provide patients with a comprehensive resource for informed decision-making, Statista and Newsweek have partnered for the first time to award America’s Best Home Health Agencies, recognizing 300 top providers across the U.S. Facilities in the 25 states with the highest number of home health agencies, according to the Centers for Medicare & Medicaid Services (CMS), were included in the survey and were ranked by state.
To create the ranking, a score was calculated for each home health agency included in the analysis.
The list is based on four data sources:
- Quality metrics data: 45% of the total score
- National online survey: 35% of the total score
- Patient experience data: 15% of the total score
- Accreditations: 5% of the total score
Quality metrics data
For each home health agency, quality metrics published by CMS were evaluated. Quality metrics were divided into two sub-groups.
Outcome measures (60% of total quality metrics score)
- Treating symptoms (30% of sub-group score)
- Preventing unplanned hospital care (30% of sub-group score)
- Preventing harm (20% of sub-group score)
- Managing daily activities (20% of sub-group score)
Process measures (40% of quality metrics score)
- Preventing harm (75% of sub-group score)
- Immunization (25% of sub-group score)
Each metric was allocated a weight, and the weighted subgroups were aggregated to produce the overall quality metrics score.
National online survey
The national online survey consisted of two parts:
- Healthcare professionals’ recommendations for home health agencies
- Quality assessment of home health agencies
a) Healthcare professionals’ recommendations
From July to August 2025, Statista conducted a nationwide online survey among healthcare professionals (e.g., nurses, physical/occupational therapists, medical doctors) and managers/administrators working in home health agencies. The survey was available on newsweek.com.
Participants were asked to recommend notable home health agencies in their state and had the option to recommend agencies in another state. Recommendations for their own employer were not allowed.
Recommendations were weighted based on the order in which they were given, with the first recommendation receiving the highest weight. Additionally, the professional experience of the participant was considered.
b) Quality assessment
For each recommendation, participants rated the recommended agency in five dimensions on a scale from 1 (“poor”) to 10 (“excellent”):
- Quality of care (30% of score)
(e.g., effectiveness of treatment plans, evidence-based care)
- Staffing (17.5% of score)
(e.g., qualifications & experience)
- Organization and accessibility (17.5% of score)
(e.g., timely visits, appointment scheduling)
- Patient counseling (17.5% of score)
(e.g., communication of staff, patient education)
- Continuity of care (17.5% of score)
(e.g., consistency of staff visits, follow-through)
A quality score was assigned to each agency based on the weighted average of these ratings.
Patient experience data
Publicly available data from patient surveys by the Home Health Consumer Assessment of Healthcare Providers and Systems (HHCAHPS) were used to analyze patient experience.
The following survey topics associated with patient care were used:
- How often the home health team gave care in a professional way
- How well the home health team communicated with patients
- How well the home health team discussed medicines, pain, and home safety with patients
- How patients rate the overall care from the home health agency
- Whether patients would recommend the home health agency to friends and family
These data were aggregated per home health agency to form the patient experience score.

To highlight agency quality and recognized expertise in home health care, three accreditations were considered in the analysis, reflecting national structural and quality standards.

Each accreditation was allocated equal weight within the scoring model.
