Abstract

107年三月底我國戶籍登記65歲以上老年人口計331萬人,占總人口14.1%,正式邁入「高齡社會」。其中,早在106年台灣已有6成的失能老人均靠家屬自行照護。縱觀先前對於行動不便者之照護輔具與其相關技術,我們發現很多設備機構過於複雜,對於單人操作方面著實有其不便性與困難度。同時,在現有的眾多發明中,鮮少有起身輔具能達到將患者從躺姿轉換到坐姿的功能,而是單純在坐姿轉乘工具之間變換。最後,本發明亦不含電類相關操作依賴,使得整體機構簡潔便利,在任何環境之中均可加以應用。 At the end of March 2017, my country's household registration registered 3.31 million people over the age of 65, accounting for 14.1% of the total population, officially entering the "senior society". 本發明姿勢轉換輔助工具之構造包含4部份:分別為(1)支撐患者背部以及倚靠的上身支撐架及背板、(2)承受患者臀部重量的坐墊板、(3)支持患者小腿的小腿支撐板、(4)便於患者立足的腳踏支撐墊及在轉換姿勢時穩固重心的重力支撐板。在每一連接面之間,分別運用樞轉裝置支持骨架運動。上身支撐架與坐墊板相連結並能相互沿第一樞轉裝置攤平或摺疊,相連結處使用可定位轉動件,例如棘輪機構、槽輪機構,故上身支撐架與坐墊板可摺疊至所需角度並固定,使其只沿單方向旋轉。使用此旋轉機構的目的為當使用者欲將患者從床上扶起時,只需單方向施力於輔具本身,其中的旋轉機構將會依齒輪結構限制反操作方向旋轉,同時支撐患者體重不致身體向後翻仰。本發明運用之樞轉裝置擁有可改變旋轉方向的功能,也就是正轉、逆轉、空轉之差異。操作者僅需調整旋鈕開關,即可使不同段落之結構隨操作者意願方向旋轉。 The structure of the posture conversion aid of the present invention includes four parts: (1) an upper body support frame and a backboard for supporting the patient's back and leaning on, (2) a cushion plate for bearing the weight of the patient's buttocks, and (3) a calf supporting the patient's calf A support plate, (4) a foot support pad for facilitating the patient to stand on, and a gravity support plate for stabilizing the center of gravity when changing postures. 由於本發明之姿勢轉換輔助工具設計主旨為輔助患者從躺姿轉換至坐姿,並移至代步工具上,因此,請先參照第1圖,此圖係本發明於患者躺姿時之結構擺放方式。初時患者平躺於床上,輔助者將患者輕抬至側臥姿勢並將本發明置於患者平躺時之位置,爾後將患者扳回,使得本發明置於患者身下。患者之臀部將置於固定之坐墊板4上方,背部將倚靠於背板1,背板1可藉由上身支撐架2來回滑動,以避免在姿勢變換的過程中患者與背板之間有相對移動。於此同時,上身支撐架2與坐墊板4相連結並能相互沿第一樞轉裝置31攤平或摺疊,相連結處使用可定位轉動件,故上身支撐架2與坐墊板4可摺疊至所需角度並固定。前述可定位轉動件(樞轉裝置3)有諸多選擇,例如使用棘輪,則可使輔助者連同本發明將患者上身扶起時,令樞轉裝置3配合患者起臥程度限制齒輪反方向旋轉,使患者輕靠於背板1而不致向後翻仰。本發明之樞轉裝置3可藉由調整齒輪轉動方向,使其依照需求順、逆時針旋轉。待患者坐定後,輔助者將患者連同本發明轉向床沿,使其腿部靠於小腿支撐板6並垂下至自然角度,理想結構圖請參照第2圖。在患者垂下小腿的同時,其膝關節處亦有樞轉裝置3,使患者不因迅速大幅度姿式變換而受傷。此時輔助者可將重力支撐板12透過第三樞轉裝
置33旋轉180度,如第2圖所示。由於有旋轉固定架9的支撐,可使重力支撐板12旋轉到達180度時不再繼續旋轉,此構造用以支撐患者與工具之重量,使之不至於在患者要從坐姿轉換為站姿時向後翻倒。當輔助者欲將患者轉換至代步工具(如輪椅)上時,本發明之結構關係圖請參照第2圖。首先輔助者先將患者連同本發明輕抬至半站立姿勢,此時透過樞轉裝置3可支持大部分患者體重,患者腳踏於腳踏支撐墊11,輔助者單腳踏於轉向踏墊10,此轉向踏墊10為可於平面自由旋轉之機構,透過萬向輪8之輔助,可使患者連同本發明旋轉至代步工具(如輪椅)方向。最終改變樞轉裝置3之旋轉方向,便可將患者至於輪椅,完成本發明輔助患者從躺姿轉換至坐姿,並移至代步工具上之目的。

動作;操作第六步如圖3-6所示,為輔助者利用第二樞轉裝置32將小腿支撐板6垂直向下擺放,協助患者小腿自然垂放。與此同時,調整第三樞轉裝置33將重力支撐板12沿第三旋轉軸16呈180度翻轉,作為站立時之重力支撐;操作第七步如圖3-7所示,為患者雙足置於腳踏支撐墊11,輔助者首先調整第一樞轉裝置31及第二樞轉裝置32的齒輪旋轉方向,使患者重心轉移,呈現半站立姿勢。爾後輔助者單腳踩於轉向踏墊10上方,使患者連同本發明沿第四旋轉軸17旋轉至載具乘坐方向;操作第八步如圖3-8所示,為輔助者調整第一樞轉裝置31及第二樞轉裝置32的齒輪旋轉方向,使患者緩慢坐於輔具上方;操作第九步如圖3-9所示,為患者最終轉換輔具成功之示意結果。
