少子高齢化や、同居家族がいても家庭内での介助者は限定的で、一人で介護を担っている場合は少なくない。在宅介護を行っている介助者の「介護うつ」が増加している。

また、ステッチ18a~18dによりマット部本体2に用いるウレタンフォーム64と表生地65とを固定し、マット部本体2を強化している。加えて、それが折り目となってマット部本体2が曲がりやすくなり抱えやすく収納もしやすくなる。

図1および図5に示すように、マット部本体2には、一端が介助者の反対側に縫い付けられ、被介助者の体を包んで保護するためのくるみ布9が設けられている。被介助者の上肢、体幹、下肢を伸縮性のある柔らかい生地でくるみ一体化し、上肢、下肢のばらつきを防ぎ、移動中の周囲へのぶつかりや引っ掛かりをなくし体を保護する。さらに、くるみ布9で包んだ体はコンパクトで体勢が崩れにくくなるので、介助者は抱き抱えやすくなり、荷重を重心で保持でき自然な姿勢で介助を行うことが可能となる。また、くるみ布9でくるむことで介助者の反対側に被介助者の体が逃げることなく落とすこともないので、介助者は自身の介助動作に集中できる。
また、体力がなく全身に力が入りにくくつかみどころのない状態の被介助者を抱き抱える場合、介助者は、落とさないように力を入れ、バランスをとるなど重さとは別のところでさらに力が必要となってくる。この時の被介助者は強く抱えられることで圧迫感や痛みを感じる。また、このような状態では、介助者は無理な体勢を強いられ負荷はさらに大きくなり、双方共に大変危険である。「持ちにくさ」を解消し持ちやすくすることで、「体重」は同じでも「感じる重さ」が軽くなる。且つ、被介助者も強く抱えられることの苦痛がなくなる。

The person being held tightly at this time feels pressured and in pain. In addition, in this situation, the caregiver is forced into an unnatural position, which puts even more strain on the caregiver, creating great danger for both parties. By eliminating the "difficulty of holding" and making the person easier to hold, the "perceived weight" becomes lighter even if the "weight" remains the same. 図2(b)~図2(d)の断面図に示すように、マット部本体表面2aのステッチ18a~18dはマット部本体裏面2bまで貫通しているが、マット部本体裏面2bに縫製された位置である図2(a)の薄い破線で表している縫い目はマット部本体表面2aには現れない。
また、マット部本体2の制作工程として、まず、マット部本体裏面2bに第1マット部ベルト28と第2マット部ベルト21,22と高摩擦部19とを縫い付ける。次に、マット部本体2に用いるウレタンフォーム64のマット部本体表面2a側に表生地65を重ね合わせステッチ18a~18dを入れて、くるみ布9を縫い付け、最後に縁取りテープ66で周部を縫い合わせるとマット部本体表面2aの表生地65が引っ張れてマット部本体2の周部のみが浮き上がる。これにより、体を包みやすく抱えやすくする効果が増す。

且つ、大腿部支持部分23および背部支持部分26は、介助者が抱き抱える両上肢に沿う位置に配置されており、介助者は両上肢による強い支持は必要としない。
また、大腿部支持部分23と背部支持部分26とは、介助者側に位置するほど互いに寄るように縫製されており、介助者の身幅に沿い抱き抱えた時にマット部本体2によれが生じることを防ぎ介助しやすいように配置されている。
また、体は部位によって密度に分布がある。この密度の大きい部位(体幹46%、大腿部14%)と両上肢16%を合わせると体重の約76%になるが、図3に示すように、第1マット部ベルト28が縫い付けられた部分は、使用時において、被介助者の体重の大部分を支えるように配置されているので、被介助者の体勢が崩れにくく安定したホールド感を可能にする。また、腰部支持部分24,25を交差して縫製し被介助者の腰部をしっかり下から支えるので、抱き抱えた時の腰部の落ち込みを防ぐことができる。

The density of the body varies depending on the part. The parts with high density (46% trunk, 14% thighs) and 16% upper limbs together make up about 76% of the body weight, but as shown in Figure 3, the part where the belt 28 of the first mat is sewn is positioned to support most of the weight of the person being assisted when in use, so the person being assisted is unlikely to lose their posture and can be held stably. The waist support parts 24, 25 are sewn crosswise to firmly support the waist of the person being assisted from below, preventing the waist from sagging when being held.

詳しく説明すると、図4に示すように、カラビナ35,36と連結部10~13とを連結した際、カラビナ35,36が左右にスライドすることで連結部10~13も連動し左右に動く。この振れ幅があることで、カラビナ35Lと連結部10、および、カラビナ35Rと連結部12が左右に動き被介助者の体の支持したいポイントに動いて保持することが出来るようになる。同様に、カラビナ36Lと連結部11、および、カラビナ36Rと連結部13も左右に動く振れ幅を設けることで介助者の身幅に沿うことができるようになる。よって、被介助者と介助者それぞれの体型に沿うように動き、被介助者の体勢を保ちながら安定した介助が行えるようになる。
また、図3に示すように、持ち手14~17は、複数人での介助時に使用するので、第2マット部ベルト21,22は、マット部本体2の短手方向端部まで縫い付け固着し把持しやすくしている。

マット部1に用いる各素材の特徴を生かして得られる効果について説明する。
マット部本体2に用いるウレタンフォーム64は、クッション性と通気性、柔軟性があり、軽く、適度なハリと厚みのある肌触りの良い素材であるので、被介助者を優しく支え、移乗先の形状や硬さを気にすることなく移乗でき、振動や衝撃を吸収し体を保護し快適に過ごすことができる。
例えば、移乗先は、検査台、車の座席やリクライニング車いす、ストレッチャー等様々であり、水平移動や上下移動を伴う移動移乗時に生じる被介助者の体への振動を軽減する。さらに、移乗先の形状(凹凸面や広さ)にマット部本体2が柔軟に変形し、居心地を損なうことなく体を保護することができる。また、リクライニング時に圧のかかる臀部の負担を和らげる。また、寒い季節には、リクライニング車いす等、座面が冷たく硬くなっているがマット部本体2に載置されたまま移乗するので防寒にもなる。
また、一人介助を行うには、扱いやすさの点からみてもマット部本体2は柔軟性があり適度なハリと厚みが必要である。被介助者の下に敷き込みやすく外しやすい。また、移動移乗時等にマット部本体2が型崩れせずに介助することができる。布状の素材は抱え上げた時等、形が崩れるので介助しづらく移乗の度に整え直す必要があり適さない。

It also relieves the burden on the buttocks that are subjected to pressure when reclining. In addition, in cold seasons, the seat of a reclining wheelchair or the like becomes cold and hard, but the transfer is made while still placed on the mat body 2, which also protects against the cold.
In addition, for one-person care, the mat body 2 needs to be flexible and have a suitable firmness and thickness from the viewpoint of ease of handling. マット部本体表面2aの表生地65に用いる滑りにくく通気性があり吸放湿機能のある肌触りの良い素材は、被介助者が、リクライニング時に体がズレ下がるのを防ぎ、長時間の臥床でも快適に過ごすことができる。
マット部本体裏面2bの高摩擦部19に用いる滑り止め加工した生地は、車やリクライニング車いす等に乗っている時にマット部1が座面からズレ下がることを防止する(図37)。
また、マット部本体裏面2bの低摩擦部20に用いる摩擦に強く滑りやすい特徴のあるトリコット素材などは、ベッド上での引き寄せや水平移乗などが楽にスムーズに行え、被介助者と介助者にかかる負担を軽減する(図38)。

背覆い部42は、ベスト部30が介助者に装着された状態で、介助者の左右肩側から左右脇側へ肩ベルト32を通すための左右肩側の開口部43(R,L)及び左右脇側の開口部44(R,L)を備えている。また、左右脇側の開口部44(R,L)の幅は、肩ベルト32が上下左右に自由に動けるような振れ幅を備えており、肩ベルト32の幅の1.5倍以上であることが望ましい。
これにより、マット部1に載置された被介助者を抱えた介助者の様々な動作や体勢に応じて、マット部1と連結した肩ベルト32が上下左右に動くことができ、荷重バランスを保つことが出来て、介助者の一連の動作の安定性を高めることができ、且つ、被介助者も安定した移乗ができる。

これにより、肩ベルト32が上下左右に自由に動く振れ幅を保ちつつ、ねじれたり暴れたりしないように交差する部分(通り道)を規制し、カラビナ36を掴みやすくし操作性を高めている。また、ステッチ47,48を施すことで、ベスト部30が強化される。
仮に、図11(c)に示すように、交差部分のみで肩ベルトを規制した場合、ベスト部の装着中は自身の後ろ側(背中側)のことは見えず、肩ベルトを整えることもできない、肩ベルトが動きすぎてカラビナを掴みにくい、肩ベルトがねじれたままカラビナを連結してしまうなど、扱いづらくなり一連の介助動作は困難になる。また、構造上、介助動作によるマット部と連結された肩ベルトの動きが、交差部分に集中してしまい破損の可能性がある。

また、図8(b)に示すように、腰部ベルト内側31bには体型の違いや介助中の腰部ベルト31のズレを防ぐために滑り止めを施している。腰部ベルトの腹側部に設けられているズレ防止部51と背部にはズレ防止素材を使ったズレ防止部50bを備えている。

また、赤カラビナ36(R,L)は、環状ゴム40で引っ掛け部53に掛止されているので背覆い部42の中に入りきることはないが、入りかけて赤カラビナ36(R,L)を掴みにくくなっても環状ゴム40を手繰り寄せて持つことができるので引っかけ部53に掛止してもしなくてもよい。
また、本発明の介助用具は、環状ゴム40を引っ掛け部53に掛止した状態で使用するが、洗濯時は引っ掛け部53から外してベスト部30から肩ベルト32を外し、肩ベルト32からカラビナ35,36を外すと肩ベルト32も洗うことができる。

また、図13に示すように、ベスト部30が支点となり重力の作用によりマット部1とのバランスを保っている。移動介助中の被介助者を介助者に引き寄せて接触した状態(図13(a))から、移乗の際の被介助者を介助者から離す(置く)動作(図13(b))までの一連の動作に対応した荷重バランスを保つことが可能になる。
また、図13(b)に示すように、前かがみになる場面では被介助者は介助者の体に接触していない状態であり介助者の腰に負担がかかる。このような時は、介助者は自身の体を支える為に、被介助者から左上肢を放し、手をついたり周囲に掴まることで腰にかかる負担は軽減され、ゆっくりと被介助者を移乗させることができる。

In such a case, the caregiver releases the left upper arm from the person being assisted and places his or her hand on the person being assisted or holds on to something around the person in order to support his or her own body, which reduces the burden on the caregiver's lower back and allows the person being assisted to be transferred slowly.
次に、ベスト部30と腰部ベルト31の構造に適した素材について説明する。
ベスト部30の内側シート29bは、10ミリ程度の厚みのあるウレタンフォーム64を用い、腰部ベルト支持部55は、くるみ布9と同じ素材である肌触りが良く伸縮性と通気性があり吸放湿機能のある素材で形成されている。肩ベルト32は第1マット部ベルト28と第2マット部ベルト21,22と同じ素材である幅50ミリ程度のシートベルトのような素材で形成され、長さ調節金具37は、登山用ハーネスなどに使用されている金具を用い、カラビナ35,36は、登山やクライミングなどで使用されるものを用いている。

さらに、ウレタンフォーム64は、介助者の体型を選ばず誰でも使用することができる柔軟さを備えている。

マット部1でコンパクトに包んで体の崩れを防いだ状態になった被介助者を、介助者は体にフィットするベスト部30と腰部ベルト31とを装着してカラビナ35,36を連結部10~13に繋ぐ。長さ調節金具37で自身に合った長さに肩ベルト32(33)を調節することで、被介助者を引き寄せ体の中心に近づけることができる。そうすることで、重さを腰で真っ直ぐ受けることができ、荷重を軽く感じられるようになり、自然な姿勢で被介助者を保持しながら移動できるようになる。同じ重さでも、重心が下にある状態より上にある状態の方が軽く感じる。仮に、介助者の方に被介助者を引き寄せられず重心で抱き抱えることができない状態で介助した場合、特に腰や肩、腕に大きな負担がかかり体勢も崩れやすくなる。そのような状態で介助される被介助者はさらに危険が増すことになる。

また、被介助者の体幹と大腿部は、第1マット部ベルト28と肩ベルト32とで保持できている(図4参照)ので、介助者は左上肢を被介助者から放すことが可能である。よって介助時の自身の不安定な体勢をとる場面(被介助者の体が介助者の体から離れた状態。図13(b)参照)では、左上肢を放し周辺に掴まるなど自身を支えて負担を軽減しながら介助が行え、被介助者を静かに移乗させることが可能となる。
結果、マット部1およびベスト部30と腰部ベルト31の組み合わせによって、一人介助はもちろん、一人介助でしか実現できない狭い空間への移動移乗もゆっくりとした動作で安定した一連の動作が可能となる。

Therefore, in a situation where the caregiver is in an unstable position while providing assistance (when the person being assisted is separated from the caregiver's body; see FIG. 13(b)), the caregiver can release the left upper limb and hold on to something around them to support himself/herself and reduce the burden while providing assistance, and the person being assisted can be transferred quietly.
As a result, the combination of the mat part 1, the vest part 30 and the waist belt 31 enables a series of slow and stable movements, not only for one person to assist the patient but also for moving into a narrow space that can only be achieved by one person to assist the patient.

更に、図10(a)に示すように、介助者の胸側の第2サブベルト33の先端部には青刺繍67が施され、脇側の第1サブベルト34の先端部には赤刺繍68が施されており、同色のカラビナ35,36が取り付けられている。第2サブベルト33および第1サブベルト34からカラビナ35,36を外して洗濯した場合、再度取り付ける時に取り付け箇所を間違わないように視認性を高めている。

次に、本発明の介助用具の活用例を説明する。
一人で抱き抱えて安定した移動移乗介助が行えるため、外部の移動手段を利用しなくて済むので経済的な負担が軽減できる。
病状が急変した時や、救急車を呼ぶ緊急性はないけれど病院に行きたい時など直ぐに病院搬送できる。早期発見早期治療につながり身体の負担が軽く済む。また、直ぐに対応できることは「安心」にもつながる。

また、夜間の救急搬送時は、診察や処置が終わればスムーズに帰宅できる。もしも、本発明の介助用具がなければ深夜に介護タクシーは利用できず朝まで待つことになる。または、介護タクシーは予約制でいつ帰宅できるかわからない。睡眠時間のとれない状態で次の日を迎えいつもと同じように活動しなければならず介助者の負担は大きくなる。
また、普段の受診の際は、病院内ではリクライニング車いす等で移動し、検査台や院内ベッドへの移乗は院内スタッフが複数人でマット部本体2の持ち手10~17を持ちマット部本体2を滑らせて水平移乗介助を行う。
このように、家のベッドからマット部1に載置された状態で来院するので全ての移乗は皆がスムーズに行える。且つ、被介助者も移乗の度の負担が軽減される。

さらに、QOLの向上は、被介助者だけでなく介助者にも同様のことがいえる。なぜなら、外部の移動手段を利用しなくても移動が自由に行えることが現実的になれば、介助者自身の意識が変わり心の視野が広がる。そういったことが介助者の心身の健康につながることになる。互いに相乗効果が得られる。
移動の自由、行動の自由は、心の自由となり、それは必ずその人の人生に変化をもたらすことに確実になる。

そして、介助者が自然な姿勢で抱えるために肩ベルト32の長さ調節金具37で長さ調節をする。目安として、抱え上げた時介助者の肘の角度が90度程度になるように肩ベルト32を調節する。
そして、図26および図27に示すように、マット部本体2の下に両上肢を入れて膝を落とし下肢の力で抱え上げ、介助者の重心に被介助者を引き寄せて保持し移動する。

次に、図29に示すように、介助者は助手席側のステップに右足を乗せて、被介助者の足側から車内へ入れる。
図30に示すように、介助者は右足に力を入れて上体を車内に入れ左手で掴まりやすいところに掴まり、右上肢で被介助者の肩から頭部あたりを支えながら移乗させる。
図31は、図30の詳細図であり運転席側から見た移乗時のイメージ図である。介助者の左手は助手席の右肩部分に掴まり自身の体を支え、右上肢は被介助者の肩から頭部あたりをマット部本体2の下から支えている。マット部本体裏面2bを通る第1マット部ベルト28が被介助者の体を下から支え、且つ、介助者が装着しているベスト部30の肩ベルト32によって、マット部1をハンモックのように被介助者を吊り下げて介助者の体全体で保持しているので左上肢を放してもそっと着座でき、安定した移乗が可能となる。介助者自身の体重移動を無理なく行うことで被介助者も安全に移乗できる。

The left hand of the caregiver holds onto the right shoulder of the passenger seat to support his/her own body, and the right upper limb supports the shoulder to head area of the person being assisted from under the mat body 2. The first mat belt 28 passing through the back surface 2b of the mat body supports the body of the person being assisted from below, and the shoulder belt 32 of the vest part 30 worn by the caregiver hangs the mat 1 like a hammock and holds the person being assisted with the entire body of the caregiver, so that the person can be seated gently even if the left upper limb is released, and a stable transfer is possible. The person being assisted can also transfer safely by shifting his/her own weight effortlessly.
図32は助手席側から見た図である。車の座席は、巻いたタオルやクッションなどを座席の窪みに敷き段差をなくし滑らかな座面に整える。足元にもクッションなどを置いて脚を伸ばせるようにする。また、足元の保護も兼ねている(図31~図33のグレーの部分)。
被介助者を車の助手席に移乗させたら連結部10,12から青カラビナ35(R,L)を外し車外に出て、連結部11,13から赤カラビナ36(R,L)も外す。又は、長さ調節金具37で肩ベルト32を伸ばし車外に出てから青カラビナ35(R,L)と赤カラビナ36(R,L)を外す。という2通りのやり方があるので介助者のやりやすい方法で分離する。
被介助者にシートベルトをして出発する(図33)。

介助者はマット部本体2の下に両上肢を入れ被介助者を抱え、被介助者の上半身から車外に出しリクライニング車いす等へ移乗する(図35)。介助者は、連結部10~13からカラビナ35,36を分離しベスト部30と腰部ベルト31とを外す。そして、マット部1に載置された状態で院内を移動する。
また、受診の際、検査台(レントゲン台、心電図等)や院内ベッドへ移乗する場合は、持ち手10~17を持つ、又は、移乗方向となるマット部本体2の片側4箇所の持ち手を持ち、院内スタッフが水平移乗介助をする(図38,39参照)。

これまでの移動移乗に関する介助用具は、被介助者を「運ぶ」ことに焦点がおかれていた。本発明の介助用具は、移動の自由を得ることによりその先にあるそれぞれの人生において「~したい」という希望を叶えるための用具として寄り添えるものである。よって、どのようなシーンにも合うデザイン性と気持ちよく過ごせる快適性を兼ね備えている。用途だけの無機質な介助用具ではなく温かみのあるデザインとする。普段の外出時や緊急時など、あらゆる場面で活用できるものである。
洋服のようなニュアンスのあるカラーバリエーションを展開し、利用者の好みの色を選べるようにする。例えば、表生地65と外側シート29aと肩覆い部41と離脱防止部45と前合わせ部52とを同一色とし、ベージュ系、ボルドー系、カーキ系、ブラウン系、ネイビー系、グレー系など数種類展開する。
また、その他の要素は各共通の色で統一する。例えば、くるみ布9はライトグレーで統一し、肩ベルト32と第1マット部ベルト28と第2マット部ベルト21,22とはライトグレーで統一し、外側シート29a側の内側シート29bと腰当部50aとズレ防止部51はブラックで統一するなど、各カラーバリエーション全体的にみても統一感のあるコーディネートをする。

図19に示すように、収納バッグ57は、袋状でありマット部1を収納可能な第1収納部58と、袋状であり第1収納部58の内側に配置されるとともに、開口端部の一部が第1収納部58の開口端部に位置する縫い合わせ部63に縫い付けられ、ベスト部30および腰部ベルト31を収納可能な第2収納部59とが一体に形成されており、第2収納部59の開口端部にはベスト部30および腰部ベルト31を収納時に開口部を閉じるためのファスナー60を備えている。それぞれを分けて収納することで互いにもつれることなく収納ができる。

最後に、困難なことの多い介護生活を送っている被介助者と介助者は、ともに大変な状況にある。その困難の中でひとつでも心配事を減らすことは重要である。「大丈夫」と思えることがあれば、介助者の安心になり、その分、心にゆとりが生まれる。本発明の介助用具は、介助者一人でも被介助者の移動移乗ができ、かつ、両者ともに身体的負担を軽減できる安全性と操作性を備えている。そして、その先にあるQOLの向上が期待できるものである。高齢者や身体の不自由な方の多様な悩みに寄り添う介助用具として、それぞれの想いの実現のために活用してほしい。不自由だったからこそ、他の人には感じられないものを感じることができると思う。不自由のなかにも自由はあるんだということ、可能性を広げ前向きになれる力になるものを見つけてほしいと願う。
本発明の介助用具は、介助者自身の経験から生まれたものであり、被介助者と支える人たちにやさしい介助用具に仕上がったと思う。
