This commit fixes the sampling rate to 40MSPS - the sampling speed
options in the GUI have no effect.

The solution for #14 (that should work with most players) is for the CAV
PIC capture state-machine to check if the player is paused during
capture.  If the player pauses but the current frame isn't the target
end-frame, the state-machine restarts the play-back and continues
capturing.

There were also a few minor bugs around handling automatic capture at
the end of discs.  These have now been (hopefully) fixed.
Note: there is a bug in the DC offset code, which is not fixed by this
commit
There was an edge-case bug where, if the sample was clipping and you had
DC offset compensation on... it would go a little wild :)  Now it simply
clips the sample correctly.

The FX3 clock has also been clocked down to 60 MHz; just to make
everything a little more tight in the electronics
