Create proposal.md outlining 40 candidate bug insertion points for a debugging/triage exercise.

This document serves as a plan for a mentoring exercise, identifying realistic, subtle, and hard-to-discover bugs in core codebase paths. The proposed bugs are designed to be difficult to catch via static analysis or trivial tests, covering categories like correctness, reliability, concurrency, and security. The proposal includes a repo map, detailed descriptions for each bug, and a top-10 recommended set.
