EOD v3.4 Review and Conversion:

After EOD v3.4 has been deployed

1. Schema Redefine: In 2018 when the EOD 2018 schemas were defined, a decision was made to put all the primary site/histology combinations (from 8800-9582) that were not AJCC eligible in the appropriate Soft Tissue Schema (based on primary site). The histologies that are NOT sarcomas need to move back to their site specific schema. The changes to logic are extensive, but affect comparatively few cases. The conversion logic includes defaults for the EOD 2018, Summary Stage 2018, SSDIs and Grade3 fields in order to eliminate the need for manual review. Please see the spreadsheet titled NAACCR 2027 Conversion Specifications and Staging Schema Redefine located on the NAACCR Implementation Guidelines page for details.

2. Colon and Rectum Schema ID: Colon and Rectum have been split for the 8th edition based schemas

   • If Date of Diagnosis Year is 2018 - 2026 and Schema ID [3800] = 00200

3. Cervical Lymph Node Head & Neck and Grade: Grade does not apply to this schema and the conversion will set all Grade fields that currently have a value to 8 for diagnosis years 2018 and later.

4. Breast and Derived Summary Grade: the calculation table for Derived Summary Grade [1975] was modified so inappropriate grades for the behavior are no longer returned. Previously coded cases should be either recalculated or converted.

5. Neoadjuvant Data Items: The three general neoadjuvant data items are being changed to be SSDIs. Neoadjuvant Therapy – Clinical Response [1633] is being lengthened to 2 characters and the codes have been revised. Some schemas do not need these data items and they will be blanked out. DCO do not need these data items as they are now SSDIs and they will also be blanked out. If Behavior ICDO3 [523] is 0, 1 or 2, these fields will be set to 7, 70, 7. Finally, if the new value for Neoadjuvant Therapy is 7, 8 or 9, and the new value for Neoadjuvant Therapy - Clinical Response is 70, 88, or 99, and separately if the new value for Neoadjuvant Therapy - Treatment Effect is 7, 8, or 9, the supporting fields will be synced to the Neoadjuvant Therapy value.

This conversion only needs to be applied in central registries that calculate the Derived Summary Stage 2018 [762].

Oropharynx HPV Associated V9 and Derived EOD 2018 Stage Group: the calculation table for Derived EOD 2018 Stage Group [818] was corrected to include calculations when Derived EOD 2018 Primary Tumor [785] = TX. This had been returning the error code 90 and such cases will need to be recalculated or converted. This conversion only needs to be applied in central registries that calculate the Derived EOD 2018 Stage Group.

15. Thyroid and Derived EOD 2018 T and Derived EOD 2018 Stage Group: for Thyroid, the Derived EOD 2018 T [785] calculation depends on Tumor Size Summary [756]. The Tumor Size break point of 040 was incorrectly assigned to the higher value and has been corrected to be T2. This also affects the Derived EOD 2018 Stage Group [818]. This conversion only needs to be applied in central registries that calculate the Derived EOD 2018 fields.
