Implemented on top of h2o/picotls#437 (implements draft-tls-esni-15).
Subsumes #3011 by @nalramli. This pull request reduces the scope to server-side behavior only, as client-side implementation requires additional work like HTTPS RR lookup. If necessary, client-side support can be added as a separate PR.
ToDo:
