The counter value in vhdx_io::{read,write} should've been accumulated
over the loop.

So far so good. I also wrote some debugging code to confirm this patch does fix the accounting issue. I think this is good to be merged and backported. |

This potentially fixes #6897. I'm running tests overnight to see if I still get the occasional failures.
