What?

Client-side media processing calls POST /wp/v2/media/<id>/finalize after every generated file has been uploaded. This PR makes a failed finalize request fail the upload instead of reporting it as a success.

Fixes #80673.

Why?

Finalize is the server's commit point: it writes the attachment metadata (the responsive sub-sizes and the final -scaled file reference). Before this change, a failed finalize was caught and only logged, and the queue item finished normally. That fired onSuccess, so:

the editor said "Media upload complete,"
the Image block was kept and could be autosaved,
but the attachment was missing its registered sizes, so the front end could not build a srcset, and the saved main-file references were left inconsistent (…-scaled.jpg in the block/metadata vs …-scaled-1.jpg in source_url).

In short, a 500 on the commit endpoint was reported to the user as a completed upload.

How?

In finalizeItem, the catch now cancels the queue item rather than falling through to finishOperation:

the error is wrapped in an UploadError (new MEDIA_FINALIZE_ERROR code + user-facing message) so it surfaces as a notice instead of a silent console.warn,
cancelItem's existing retry guard already skips retry once an attachment id exists (which is always the case at finalize), so this does not loop or re-upload,
finalize only runs after all child sideloads have been removed, so cancelling the parent leaves no orphaned queue items.

Testing Instructions

Automated:

npm run test:unit -- packages/upload-media/src/store/test/private-actions.js -t "finalizeItem"

Manual (from the issue): with client-side media processing active (crossOriginIsolated + SharedArrayBuffer), add an mu-plugin that forces /wp/v2/media/<id>/finalize to return a 500, then upload an image through an Image block.

Before: editor shows "Media upload complete" and keeps the image.
After: the finalize failure surfaces as an upload error notice; no false success.

Known limitations / open questions for reviewers

This stops the false success but does not remove the image from the block. The upload step already fired onChange with the real attachment id/url before finalize, so the block committed those first. The main file is a valid attachment, but the missing sub-sizes / mismatched file references persist in the saved post. Fully reverting the block on finalize failure would be a larger, separate change.
No retry for a transient finalize 500 — the current retry mechanism re-runs the whole item (including re-upload), so it can't cleanly retry finalize alone. This PR fails fast instead.

The meat. Stop swallowing the finalize failure.

Instead, cancelItem runs, which fires onError (surfacing an error notice) and never lets onSuccess tell us that the upload is complete.

This is testing well for me so far!

Overall I think this approach is likely fine for a 7.1 fix, but it seems there's some nuance to it.

One challenge is that the success message for the snackbar appears to fire when the queue is finished, even if the item was cancelled. We probably don't need to fix that here, but just noting that I saw it at least!

The other challenge is that we clear out / cancel the image, and that puts the image back into the placeholder state. But, the unfinished/unfinalized image still appears to be in the media library. I think it's probably safer to leave it there in its unfinalized state than try to delete the image after a failed finalize request... but it does mean things are sort of in an in-between state for this failure.

What do you reckon? I'm leaning toward going with this PR as I think it's a better and clearer state than on trunk and the empty placeholder will encourage the user to attempt to re-upload rather than use an unfinished image.

I think I'd keep the console.warn() line here. I notice in trunk that it provides a more detailed overview of what failed, and I don't think cause is actually exposed anywhere?

Another alternative would be to use debug which only fires in development / debugging environments. I think I'd still lean toward keeping console.warn() as it means that in any environment it's a little easier to see what's going on. Here's trunk:

Something that came up while taking this for a spin with Claude: it turns out apiFetch doesn't return error as an instance of Error, but as a plain object. How should we handle it?

Note that I think cause is only used internally to this package, so we might not need to do much about it in this PR. Basically I think we just need to make a decision about how we do or don't want to use non-Error errors 😄

The UI will get a generic error, but like you say, we should reinstate the console.warn for the raw error.

Yeah that seems like existing behaviour. I suppose the snackbar can’t tell that a failure happened 🤔

Yeah, you're right, it seems to be a simple change to add it in, and we have a precedent in that we also already do it when sideload items fail:

That fired `onSuccess`, so the editor reported "Media upload complete" and kept a block whose attachment is missing its registered sizes (no srcset on the front end) with inconsistent file references, and could then autosave it. Cancel the item on a finalize failure instead, wrapping the error in an UploadError so the failure surfaces to the user rather than being reported as success. Fixes #80673.

Address review feedback on the finalize failure handling: - Reinstate the `console.warn( 'Media finalization failed:', error )` call so the underlying failure stays visible in every environment. The user-facing notice is deliberately generic, so the console log is the only place with the full detail. - Drop `cause` from the UploadError. `apiFetch` rejects a failed REST request with a plain object rather than an Error instance, so the `error instanceof Error` guard left `cause` undefined for the real failure, and `cause` is not surfaced outside this package anyway. Update the test to reject with a plain object, matching apiFetch, and to assert the warning is logged.

Alrighty, just taking a look at this again with the delete in there, and I think I'm going to go back on some of the things I said the other day, I'm sorry!

After interrogating the idea a bit in a Claude session, I'm a little wary of the different ways that deleting the attachment would be the wrong approach (i.e. finalize succeeded, but there's a gateway timeout and so on), and I remember now that one of the things that I think @adamsilverstein wanted to look at separately at some stage was being able to re-run (or reprocess) existing items. Also, it's possible that if finalize fails then a delete might also fail!

If we are to delete on failure, I think we might want a slightly more complex approach where we only do so based on certain kinds of error codes, or after repeated attempts to apply finalize subsequent times.

So, I'm wondering if the best fix for now is closer to what you had before (i.e. basically reverting bea1248) — don't delete the attachment, but we cancel the upload so the image block goes back to its placeholder state.

What do you reckon?

Thanks for the back-and-forth here. Re-tested and uploads are working fine as before, and if I add an mu-plugin to fail the finalize endpoint, then the Image block gets cleared out and I can see the error in the console:

This looks like a good place to land this fix for 7.1 to me, and when Adam's back we can always follow-up if he has other thoughts or plans.
