Changed the scripts to read a stream from SQL Server to use an index seek where possible. Both V2 and V3 have been updated. Switched to clustered index in V3 schema and removed obsolete index.

Optimization - Lifts the core metadata, MaxAge and MaxCount to the streams table. This allows this data to be read at the time of reading a stream instead of requiring an additional query. Fixes #112.
