Am no expert in this area, yet your post to the community deserves a response.
Seems that to switch hearing-aids effortlessly between Apple devices / computers, they would need to be signed in to the same Apple Account ... wondering if you may have different Apple Accounts for work and for home ?
Else if you use the one Apple Account with each of the two computers : there may be accessibility settings you can use:

Change Hearing Devices settings for accessibility on Mac - Apple Support
Eg: says
"Use Hearing Devices settings to pair Made for iPhone (MFi) hearing aids or sound processors with your Mac and adjust their settings."
"To change these settings, choose Apple menu
> System Settings, click Accessibility
in the sidebar, then click Hearing Devices. (You may need to scroll down.)
Option: Available Devices
When unpaired hearing devices are in range of your Mac, they appear in the list of available devices. To pair hearing devices with your Mac, move the pointer over the devices, then click Connect. See Pair hearing devices with your Mac.
Option: Control Nearby Devices
When your hearing devices are paired with and connected to another device where you’re signed in to your Apple Account, use Hearing Devices settings on your Mac to adjust the settings for that device (such as an iPhone)."
Wondering whether or not you are using "handoff" between devices, and if that can help:.
Use Handoff to continue tasks on your other devices - Apple Support

Use Handoff with any Mac, iPhone, iPad or Apple Watch that meets the Continuity system requirements. Handoff works when your devices are near each other and set up as follows.
- Each device is signed in to iCloud with the same Apple Account. To see the Apple Account used by Apple Watch, open the Apple Watch app on your iPhone, then go to General > Apple Account (or Apple ID).
- Each device has Bluetooth and Wi-Fi turned on.
- Each device has Handoff turned on:
- Mac with macOS Ventura or later: Choose Apple menu > System Settings, click General in the sidebar, then click AirDrop & Handoff on the right. Turn on "Allow Handoff between this Mac and your iCloud devices".
